When you place it under your head, neck and shoulders, it disperses pressure avoiding any pinch points. Your shoulders and neck experience a deep release that transitions up toward the top of your skull.

Will it hurt? Yes. But only for a little in the beginning.
Each spike responds to tension in your body, so more tension = more sensation. The initial “Ouch!” you feel activates your body’s pain response and tells it to send happiness hormones and pain-relief chemicals to the area you’re targeting. I like using the pillow sometimes in addition to my mat. I think if I were to purchase again, I would get the classic to be able to feel it more on my neck. The quality is lovely!

Nice, but I don't think necessary
I bought the pillow and the mat thinking they would go nicely together. I haven't really felt much with the pillow because my hair shields my skull from feeling the sensation. If I were to do it all over again, I would have gotten a level 3 pillow with my level 2 mat
