When you place it under your head, neck and shoulders, it disperses pressure avoiding any pinch points. Your shoulders and neck experience a deep release that transitions up toward the top of your skull.

Will it hurt? Yes. But only for a little in the beginning.
Each spike responds to tension in your body, so more tension = more sensation. The initial “Ouch!” you feel activates your body’s pain response and tells it to send happiness hormones and pain-relief chemicals to the area you’re targeting. If those spikes aren’t sharp enough – or you’re beyond relaxed already – you won’t feel much.

Surprisingly Addictive
I did not expect that I’d long for something painful (at first), but the Shakti mat + pillow completely changed my mind. What started as curiosity quickly turned into a meditation routine I actually look forward to. The first “bite” fades away quickly and turns into deep relaxation. I’ve had my mat for two weeks now, and I’ve noticed that this relaxation process starts much faster than when I first began. The only downside is that I can’t hit every spike on the pillow with my head, neck, and back ;)

This pillow is great. I have the classic (roll) Level 1 pillow but I find this premium pillow (which I got at Level 2) has great coverage and is more comfortable for my head/neck.
